My cat had exposure to FIV around 4 years ago and is lethargic, losing weight, and stumbling when walking.

Poor Sheriff! Chances are this has nothing to do with the FIV exposure, but I would definitely have him tested for it. There are many other things that can cause lethargy, weight loss and stumbling including high thyroid, nutritional deficits, toxins, spinal problems, certain infections and much more. I would have your vet take a look at him. Your vet will be able to test him for FIV, do other bloodwork, and even x-rays if needed. Your vet will be able to get him started on the proper meds to help him feel better. I hope this helps!
